COMMUNITY IMPACT

━━━━━━━━

This project will provide a critical safety net for individuals transitioning from treatment facilities, the justice system,
or unstable housing situations.

By offering structured case management and housing assistance, we aim to:

● Reduce homelessness and housing insecurity among individuals in recovery.

● Lower relapse rates by providing stable, supportive environments. Increase employment rates and financial stability through job readiness programs.

● Strengthen families by offering parenting support and reunification resources.

● Enhance community well-being by reducing the strain on emergency services and shelters.

FUNDING NEEDS & UTILIZATION

We are seeking federal funding to:

● Expand the number of transitional housing units available.

● Hire additional case managers to provide individualized support.
Develop and implement structured life skills and workforce training programs.

● Cover operational costs for maintaining safe and supportive housing facilities.

● Strengthen partnerships with local organizations to enhance wraparound services.

TARGET POPULATION

● Individuals in early recovery from Substance Use Disorders

● Women and women with children facing homelessness or housing instability

● Couples in recovery seeking stability together

● Low-income individuals in need of structured transitional housing

WHY THIS MATTERS

● Substance Use Disorder recovery is a long-term process, and many individuals face housing barriers that put them at risk for relapse or returning to unstable environments.

● Resilient Pathways: Transitional Housing & Recovery Support ensures that individuals and families receive the guidance, stability, and support they need to successfully rebuild their lives.
